LACUYES (Lactulose Concentrate Solution)
Description
LACUYES contains Lactulose, a synthetic, non-absorbable disaccharide widely used as an osmotic laxative. It is particularly effective in managing chronic constipation and preventing/treating hepatic encephalopathy by reducing systemic ammonia levels. Suitable for long-term use due to its non-habit forming nature.
Mechanism of Action (MOA)
- Reaches colon unchanged (not digested in small intestine)
- Fermented by gut flora → lactic acid, acetic acid
- ↓ Colonic pH → converts ammonia (NH₃) to ammonium (NH₄⁺)
- Osmotic effect → increases water retention in stool
- Stimulates peristalsis → promotes bowel movement
Pharmacokinetics
- Absorption: Negligible systemic absorption
- Onset: 24–48 hours
- Metabolism: By colonic bacteria
- Excretion: Feces
Uses
- Chronic constipation (first-line osmotic laxative)
- Hepatic encephalopathy (reduces ammonia toxicity)
- Portal-systemic encephalopathy
- Pre/post-operative bowel regulation
Dosage (General)
- Constipation: 15–30 ml daily (adjust based on response)
- Hepatic encephalopathy: 30–45 ml 3–4 times/day
Side Effects
- Common: Flatulence, bloating, abdominal discomfort
- Dose-related: Diarrhea
- Long-term: Electrolyte imbalance (Na⁺, K⁺ disturbances)
Precautions
- Monitor electrolytes in elderly & long-term use
- Use cautiously in diabetics (contains sugars)
- Avoid in galactosemia
- Ensure adequate fluid intake
Drug Interactions
- Antacids may reduce efficacy
- Antibiotics may alter gut flora → reduced effect
